A patient's care plan is often overloaded with information. Many electronic health record systems and care management platforms seek to simplify care planning display without obfuscating crucial information. I assisted Cityblock Health in redesigning a simpler patient Care Plan. This included simplifying display of interactions between patients & care-takers, care goals & care plan tasks.

Role: one of three engineers in the project.
