Millions of people across the world live their everyday with chronic illnesses. These persistent conditions often involve complex treatment plans & require more frequent clinical touch-points. Additional clinical interactions can help increase treatment plan adherence & lower avoidable hospitalizations. In order to increase cadence of patient touch-points, it's often helpful to lean into telemedicine. The Chronic Care Manager generated a list of patients for telemedic enrollment (based on commorbidities & payer eligibility). It also queued up patients for follow up phone conversations based on a variety of variables. It streamlines CCM enrollment and management.
